· ITEM #8, MARK TOURISM BOX.
· ITEMS #9, #16, #18 AND #39 LEAVE BLANK.
· ITEM #10 MAY BE COMPLETED WITH THE NAME OF THE INITIAL CITY OF YOUR ARRIVAL IN RUSSIA.
· DATE AND SIGN BOTTOM OF THE FORM.
